Perimenopause

10 early signs of perimenopause nobody tells you about

Reviewed by our medical team4 minute readUpdated month 2026

You feel different, but you can't quite say why. Maybe you're more tired, more anxious, or your periods aren't what they used to be. If you're in your late 30s or 40s, it could be perimenopause, the years leading up to menopause when your hormones start to change.

Most women expect hot flushes. But the first signs are often much quieter, and easy to miss.

  1. Your periods change

They might come closer together, further apart, or be heavier or lighter than usual. Often this is the very first clue.

  2. You feel anxious for no clear reason

Many women notice new anxiety or feel on edge, even when life is calm.

  3. Brain fog

Forgetting words, losing your keys or walking into a room and forgetting why. It's common, and it's real.

  4. You're waking up at 3am

Falling asleep is fine, but staying asleep isn't. Changing hormones can disturb your sleep, even without night sweats.

  5. Your PMS gets worse

Mood swings, irritability and tearfulness before your period can become much stronger in your 40s.

  6. You're tired all the time

Not just busy tired. A deep tiredness that rest doesn't fix.

  7. Aches and stiff joints

Many women are surprised to learn that aching joints can be linked to hormone changes.

  8. Weight shifts to your middle

Even if you're eating the same, you may notice weight settling around your tummy.

  9. Low mood or loss of confidence

Feeling flat, or less like yourself, is one of the most common signs, and one of the most overlooked.

  10. Less interest in sex

Falling hormones, tiredness and stress can all lower your libido.

Did you know?

Iron contributes to the reduction of tiredness and fatigue. Heavy periods in perimenopause can leave you low in iron, which is worth checking.

When to get help

If these changes are affecting your work, your relationships or how you feel about yourself, it's worth talking to someone. And if you have bleeding after sex, between periods or after your periods have stopped, please see a doctor promptly.
